How to Request Online-Eligibility/Examination/Authentication Record for Civil Service Passers | CSC Region IV.

ON-LINE REQUEST FOR ELIGIBILITY/EXAMINATION RECORDS FOR CAREER SERVICE EXAMINATIONS CONDUCTED BY CSC REGION 4 for 2010 - onwards (For Passers/Eligibles only).


Note: (Processing of request of records for recently conducted examinations will be available within two (2) months after the release of the results.)


CERTIFICATION OF ELIGIBILITY (Who may avail of the service?)
1.  Those who lost their Certificate of Eligibility (due to typhoon, flood, fire, theft, etc.)
2.  Those who are not in possession of their Certificate of Eligibility (did not receive/
      claim their certificate, submitted CoE to the agency, etc.)
3.   Those who want to replace their Certificate of Eligibility (old/torn/worn-out certificate,
       request for correction of personal information has been duly granted by the
       Commission, etc.)

List of Requirements for Certification of Eligibility
1.  Properly accomplished Eligibility/Examination Records Request Form (ERRF)
     and Declaration Form (DF)
2.  One piece 1 inch by 1 inch picture with full name tag and signature affixed prior
      to having the photograph taken
         > Signature must be on top of the printed name.
         > Photograph should have been taken within three (3) months prior to filing of
             request for Certification of Eligibility.
         > Scanned, computer-generated photo/name/signature will not be accepted.

3.  Certification fee: Php 100.00

4.  Two (2) valid Identification (ID) Cards or if the requesting party works/lives abroad:
         > Copy of passport; and
         > Copy of one (1) Identification Card
                Both duly authenticated/validated by the Philippine Embassy or Consular Office.

Note: Valid ID contains applicant’s clear picture, date of birth, signature, and signature of the authorized head of the issuing agency such as Driver’s License, Passport, PRC License, SSS ID, GSIS ID (UMID), Voter's ID/Voter's Certification, BIR/Taxpayer's ID, Philhealth ID, Company/Office ID, School ID, Police Clearance, Postal ID, Barangay ID, NBI Clearance, Seaman's Book, HDMF Transaction Card, PWD ID, Solo Parent ID, Senior Citizen's ID, and Alien Certificate of Registration Identity Card.  (Any ID not included in the list shall NOT be accepted.)

Additional Requirement:
PSA-Issued Marriage Contract for women who married after taking the examination.

AUTHENTICATION OF CERTIFICATE OF ELIGIBILITY (Who may avail of the service?)
Eligibles who have original copy/ies of the certificate/s of eligibility or report/s of rating in their possession and want it/these authenticated.

List of Requirements for Authentication of Certification of Eligibility
1. Properly accomplished Eligibility/Exam Records Request Form (ERRF)
2. Original Certificate/Certification of Eligibility or Report of Rating
3. Authentication fee: Php 50.00 per copy
4.    Two (2) valid Identification (ID) Cards or if the requesting party works/lives abroad:
          > Copy of passport; and
          > Copy of one (1) Identification Card
                Both duly authenticated/validated by the Philippine Embassy or Consular Office.

Note: Valid ID contains applicant’s clear picture, date of birth, signature, and signature of the authorized head of the issuing agency such as Driver’s License, Passport, PRC License, SSS ID, GSIS ID (UMID), Voter's ID/Voter's Certification, BIR/Taxpayer's ID, Philhealth ID, Company/Office ID, School ID, Police Clearance, Postal ID, Barangay ID, NBI Clearance, Seaman's Book, HDMF Transaction Card, PWD ID, Solo Parent ID, Senior Citizen's ID, and Alien Certificate of Registration Identity Card.  (Any ID not included in the list shall NOT be accepted.)

PROCEDURE IN AVAILING THE SERVICE
STEP 1            -   Client to accomplish this on-line form;
STEP 2            -   Download and accomplish the Examination Records Request Form (ERRF)
                              and the Declaration Form;

STEP 3            -   Upload the photo or scanned copy of the following:
                                     a. ERRF;
                                     b. Declaration Form;
                                     c. 2 Valid ID Cards;
                                     d. PSA-Issued Marriage Contract for women who married
                                          after taking the examination;
                                     e. 1 inch by 1 inch picture with full name tag and signature
                                            (for certification of eligibility); and
                                     f. Proof of Payment (Official Receipt issued by the CSCRO4
                                         Field Office or bank validated deposit slip)

STEP 4            -  Pay the processing fee in any CSCRO-4 Field Offices or
                             Deposit the processing fee at any Land Bank of the Philippines Branches.

              (Reminder: Please be informed that the bank charges interbranch deposit fee)

STEP 5            - Send the original copy of the requirements as mentioned in Step 3 together
                            with the original copy of official receipt issued by any CSCRO-4 Field Office
                            or the bank validated deposit slip as proof of payment of the processing
                            fee including the prepaid self-addressed return envelope from any courier
                            of your choice to  :

                                      EXAMINATION SERVICES DIVISION
                                      Civil Service Commission Regional Office 4
                                      139 Panay Ave., South Triangle, Quezon City
                             
 STEP 6            -  Requested examination records and the official receipt will be released
                              to requesting party within three (3) working days upon receipt of the
                             complete documents mentioned in Step 3 and 5 using the prepaid
                             self-addressed envelope.

IMPORTANT REMINDERS: PLEASE DO NOT FORGET TO SEND US YOUR 2 ORIGINAL VALID IDENTIFICATION CARDS TOGETHER WITH THE ACCOMPLISHED EXAMINATION RECORDS REQUEST FORM, DECLARATION FORM  and OTHER REQUIREMENTS.
