Scope and Coverage:

• DILG officers - Data Encoding and Uploading
• LGU HRMOS - Submit the hard copy sealed envelopes to DILG Officers
• Elective Officials - accomplish the ELOPDS from DILG Field Officers

Reminders:

1. Use BLUE Ball pen or BLUE Font

Write in "CAPITAL LETTERS"

3. PUT "N/A" for Field not applicable

4. Follow the "MM/DD/YYYY"

5. Fields with Asterisk (*) is Mandatory

6. Physical Picture: Passport Size, White Background with Signature over Printed Name

7. Printed in Legal Size Paper
