1. If not yet register, click here or copy + paste this link to your browser: https://memberinquiry.philhealth.gov.ph/info/validate.php
- In "
Registration" complete
Basic Information (
use e
mail address that is currently active) and
Security Questions (take note to remember, you'll use this upon sign-in later on this tutorial).
- Once done click
Submit Registration button at the bottom part.
- Check your email inbox. Open the email and take note of your PIN and PASSWORD.
Then click the link that prompt you to activate your account to complete your registration:
2. Now, go back to this link:
https://memberinquiry.philhealth.gov.ph/ (
online services > member inquiry) and then sign-in (
Your PIN also corresponds to your PHILHEALTH ID). Then Write your the answer to your
security questions.
3. You can see now your PHILHEALTH Static Information. To check your
CONTRIBUTIONS, just select
PREMIUM CONTRIBUTIONS. To print
PHILHEALTH MDR, just click
MDR PRINTING and; to
CHANGE PASSWORD, click
USER SETTINGS and go to the bottom part. To
LOGOUT click
LOGOUT.

Kukuha ako ng csf ko kasi po manganganak ako hindi po ako makalog in,ano gagawin ko yung sa asawa ko gagamitin ko pong philhealth eh sa goverment po yun and exprired na po yun 2013 pa kasi yun,tnx po sa sasagot
ReplyDeleteNagbayad na kami mula Jan. to June contributions ngunit hindi pa kami nakatanggap ng Receipt, paano namin makukuha yon, at ano ang gagawin namin?
ReplyDeleteYou can go to the nearest Philhealth Branch and ask them to kindly check whether your contributions made online reflected on your PHILHEALTH account. Or if you already have account online, just sign in here: https://memberinquiry.philhealth.gov.ph/member/ and you will find your premium contributions. I hope I provided some help.
DeleteBarangay Officials Philhealth contribution po! 6 mos. po ang binabayaran namin online pero hindi namin mahanap kung paano makukuha ang receipt through online po! Sana matulungan ninyo kami!
