If you will be filing for the Petition for Change of Registered Name due to Marriage and/or Petition for Correction of Entries or Data, please proceed to the PRC Central Office or to the nearest Regional Office to file said petition/s.
Once your petition has been approved, you can proceed with the renewal of your Professional Identification Card in this website.

NOTE: The maximum processing time for the Petition for Change of Registered Name due to Marriage and/or Petition for Correction of Entries or Data is two (2) months or 60 days from receipt of complete documents.

  • Download Form for Petition for Correction of Entries or Data
  • Download Form for Petition for Change of Registered Name due to Marriage


  • Step 2. Pay for metered documentary stamps at the CSC (Customer Service Counter). Please make sure that your photo/xerox copy/ies of documents have metered documentary stamps.
    Step 3. Pay prescribed fees at the Cashier.
    Step 4. Present duly accomplished AS together with documents and receive your claim slip at the designated Registration Division Window
    Step 5. Claim your documents as scheduled. Please refer to your claim slip for further instructions.

    Requirements:
    • Duly accomplished Action Sheet
    • Original and photo/xerox copies of professional license or registration certificate to be authenticated with metered documentary stamps
    • Official receipt
